The following steps should be taken to begin the process of dividing retirement accounts per a Judgment of Divorce:

1. The intake form must be fully completed, click here to reach the form.  Once completed the form must be sent to the address listed below.
2. The Judgment of Divorce should be e-mailed faxed or mailed to us at the address below
3. A copy of the most recent statements for each account to be divided must be sent to the address listed below
4. The flat fee must be paid in full.

Please note that all personal information given will be kept under lock and key.  No personal information will be shared with anyone other than the Plan Administrator, your lawyers, or our staff.

If your Judgment calls for a split of our fee with your ex-spouse, we will request the payment, but we will not take any steps to enforce the payment.

NO WORK WILL BEGIN UNTIL ITEMS 1-4 ABOVE HAVE BEEN COMPLETED

WE WILL DESTROY YOUR FILE TWO YEARS AFTER THE ORDER IS ACCEPTED BY THE PLAN OR TWO YEARS AFTER YOUR LAST COMMUNICATION WITH US.
